Our new primary care services include routine checkups, preventive screenings and chronic condition management
Primary Care
We are excited to have a multidisciplinary team of Certified Nurse Midwives and Family Nurse Practitioners, all located within Dar a Luz, who can address health concerns and questions from teens, through adulthood, and into menopause. Patient-centered care is at the heart of everything we do. We believe healthcare should be built around your goals, preferences, and unique health needs.
Our primary care team takes the time to listen, then answer questions to support you and your overall well-being. Our priority is to ensure that you are involved in every decision. By combining compassionate communication with personalized care plans and coordinated services, we strive to make your experience respectful and empowering. Here, you’re not just a patient—you’re an active participant in your health journey.
At this time we are not offering primary care for men and kids under the age of 12 as well as same-day urgent care such as seasonal illness/rash/injuries.
Routine Checkups
Routine adult checkups are a cornerstone of staying healthy and catching potential concerns early. Our extended visit times allows the primary care team to review your medical history, monitor vital signs, and offer a physical exam. We take time to discuss your overall well-being—addressing nutrition, exercise, stress, and preventive care to help you maintain a healthy lifestyle. One benefit of working with our office is that we have an extensive referral network to support your access to community resources such as nutritionists, mental health specialists, physical therapists, body workers, acupuncturists and more to manage your health concerns.
Preventative Screenings
We can provide recommended screenings based on your age, lifestyle, and risk factors including, but not limited to:
- Diabetes
- Blood pressure
- Cholesterol
- Sexually transmitted diseases
- In-house screening for cervical cancer
- In-house screening for mental health including anxiety and depression
- Referrals for vision and hearing screening
- Referrals for cancer screening including skin, lung, breast, and colon cancer screening
Chronic Disease Management
Our primary care team can work with you to manage chronic conditions such as hypertension, type 2 diabetes, hyperlipidemia, obesity, and hypothyroidism, that are responsive to first-line therapies. This means that they can be well-controlled by first line medications, and thus not need the expertise of an internal medicine physician. We will be able to connect you with a specialist if your condition(s) requires the next level of care.
In the event that you happen to be a woman in the perimenopause transition, it is beneficial to work with a team that understands the interplay of your changing hormonal landscape and the risk and/or progression of various chronic conditions.
